What Is Eyelid Surgery?

Eyelid surgery, or blepharoplasty, is surgery to repair droopy or puffy eyelids by removing excess skin and fat and tightening the eyelid muscles. It is performed on the upper eyelids, lower eyelids, or both. Eyelid surgery can be conducted in conjunction with a brow lift to correct drooping eyebrows or with a facelift to correct sagging in the lower part of the face.

An upper eyelid lift, or upper blepharoplasty, corrects drooping or hooding in the upper eyelids. Droopy upper eyelids can make you look older than you are and more tired than you feel. The extra effort required to raise the upper eyelids can result in forehead creases and can make reading tiring. An upper eyelid lift corrects these problems and restores a more attractive look to your eyes and face. The surgery is performed with an incision hidden within the natural fold of the upper eyelid.

Lower eyelid fat removal, or transconjunctival blepharoplasty, removes bags and puffiness below the eyes to make you look less tired and more energetic. The procedure uses an incision made inside the lower eyelid, resulting in no visible scarring.

A lower eyelid lift, or lower blepharoplasty, lifts a droopy lower eyelid. It is performed using an inconspicuous incision along the lash line and smile creases of the lower eyelid. Excess muscle and skin are removed to raise the lower eyelid, and fat is removed or repositioned for a naturally youthful and attractive appearance. [The skin below the eye may be further tightened and smoothed using laser skin resurfacing or chemical peels.]

How Is Eyelid Surgery Performed?

Eyelid surgery is usually performed using local anesthesia and mild sedation. If the changes to be made are extensive or if the surgery is combined with another procedure, such as a brow lift or facelift, general anesthesia may be used. As part of your consultation, Dr. Loizides will explain the surgical approach he proposes for your procedure.

Depending on the changes to be made, the incision may be made:

  • Within the crease of the upper eyelid (for an upper eyelid lift)
  • Inside the lower eyelid (for lower eyelid fat removal)
  • Along the lash line and smile creases of the lower eyelid (for a lower eyelid lift)

Excess skin and fat are removed, and loosened muscles are tightened to lift and thin the eyelid. Fat may be repositioned for a naturally attractive appearance. Fine sutures are used to close external incisions in a way that minimizes visible scarring. 

The procedure typically takes between 45 minutes and 2 hours, depending on the changes to be made. After the surgery is complete and you are fully alert, you will be able to go home. You should arrange ahead of time to have a friend or family member drive you home when you are discharged and stay with you for the first night.

What to Expect During Eyelid Surgery Recovery

You will need to stay home from work and limit your activities for several days after surgery to give your eyelids time to heal. Applying ice packs or cold packs during the first two days can help reduce swelling or bruising. After that, warm compresses can help to promote healing. Some people experience dry eyes in the first few days after surgery, which can be relieved with eye drops or artificial tears. 

It may take a few weeks for healing to complete. During this time, you should not wear eye makeup. You’ll also need to forgo contact lenses at first, while your eyelids are swollen. When the swelling subsides and it is again comfortable to do so, you can resume using contact lenses.

The results of an upper or lower eyelid lift are very long-lasting. The fat that is removed is gone forever and will not come back. The same is true for the skin that is removed to lift the eyelids. Your skin will continue to age, however, and some loosening could recur in 15 to 20 years. If this happens, you might choose to have a touch-up procedure.

No. Eyelid surgery corrects looseness and puffiness in the eyelids and the area just below the eyes. The surgery won’t affect wrinkles or lines on the sides of your eyes. Crow’s feet can be smoothed with injections of Botox® or dermal fillers.

The fat removed in lower eyelid surgery can reduce the appearance of bags under the eyes, but it won’t change the coloring of dark circles under the eyes. The best remedies for dark circles are healthy sleep habits, cold compresses, and the application of bleaching creams, cucumber slices, or tea bags to the area below the eyes. A light chemical peel can also reduce their appearance.

There’s no “best” or “right” age for eyelid surgery. Younger people may choose to have an eyelid lift to improve their appearance if they have naturally puffy or droopy eyelids. People in their 40s, 50s, and older may choose the procedure to correct age-related changes. The best age for your eyelift is when you need it.

Drooping eyebrows can make the upper eyelids appear to sag. In your consultation, Dr. Loizides will examine you to determine whether the problem is with your upper eyelids or laxity in your brows. You can get a sense of this by placing your fingers just above your eyebrows and gently raising them to an attractive position. If this shift corrects the sagging in your upper eyelids, the solution for you may be a brow lift.
