Breast Implant Placement: Over vs. Under the Muscle. Which Is Right for You?

Over- vs. Under-the-Muscle Breast Implant Placement: What's the Difference?

Breast implants can be positioned either above or below the pectoral muscle. Over-the-muscle placement, also called subglandular placement, positions the implant behind the natural breast tissue but in front of the chest muscle. Under-the-muscle, or subpectoral, placement positions at least part of the implant beneath the pectoralis muscle.

Neither option is automatically better. Each has advantages depending on your natural breast tissue, chest anatomy, implant size, desired appearance, and lifestyle.

What Does Breast Augmentation Look Like for Over- and Under-Muscle Implant Placement?

Breast augmentation is typically performed under general anesthesia. Dr. Loizides creates an incision based on the planned implant and your anatomy, commonly along the breast crease or around the edge of the areola.

For over-the-muscle placement, Dr. Loizides creates a pocket between the breast tissue and pectoral muscle. The implant is carefully positioned within this space and centered behind the nipple.

For under-the-muscle placement, the pocket extends beneath the pectoral muscle, so the muscle provides additional coverage over the implant. Once the selected implant is properly positioned, the surgeon closes the deeper tissues with dissolvable sutures and applies sterile dressings and a compression bra.

Breast implant surgery usually takes about an hour, although surgery can take longer when combined with other procedures, such as a breast lift or liposuction. Patients generally return home the same day and should arrange for someone to drive them and remain with them during the first night.

How Does Breast Implant Placement Affect Implant Feel?

Placement can affect how noticeable an implant feels beneath the breast tissue, although implant material, existing tissue thickness, and implant size also matter.

Under-muscle placement provides an extra layer of soft-tissue coverage over part of the implant. This can be particularly helpful for patients with thinner natural breast tissue because it may make the implant edges less noticeable. It can also create a smoother transition along the upper breast.

Over-muscle placement relies primarily on the patient’s existing breast tissue to cover the implant. Patients with enough natural tissue may still achieve a very soft, natural feel with this approach.

The implant itself also matters. Cohesive silicone gel implants generally provide a softer feel that more closely resembles natural breast tissue than saline implants. During consultation, Dr. Loizides evaluates placement alongside implant type, width, profile, and volume because all of these choices work together to influence the final result.

Which Breast Implant Placement Option Has an Easier Recovery?

Over-the-muscle placement can sometimes have an easier early recovery because the chest muscle doesn’t need to stretch over the implant. Patients may experience less muscle-related tightness and discomfort during the first several days.

Under-muscle placement can create more pressure or tightness across the chest early in recovery because it affects the pectoral muscle. This doesn’t necessarily mean recovery will be difficult, but the first stage may feel different.

Regardless of implant position, Dr. Loizides generally advises patients to expect:

  • About 1 to 2 weeks away from work, depending on job demands
  • Light walking during early recovery
  • Low-impact exercise after approximately two weeks when cleared
  • A gradual return to more strenuous activity around 4 to 6 weeks
  • Continued use of a supportive bra as recommended

Individual recovery varies, and implant placement is only one factor. Implant size, combined procedures, overall health, and how closely you follow post-operative instructions also affect healing.

Does Breast Implant Placement Affect the Breast Appearance?

The placement of your breast implant can influence upper-pole fullness, implant visibility, movement, and how gradually the breast transitions from the chest wall.

Under-muscle placement often provides more coverage through the upper portion of the breast. This can be useful for patients with limited natural tissue who want to minimize visible implant edges or rippling. The additional coverage can also help create a softer transition.

Over-muscle placement may be appropriate for patients who already have enough breast tissue to cover the implant well. Because the implant sits directly beneath the breast, this option can also avoid the movement or distortion that may occur when strong chest muscles contract over an implant.
